The first physician to actually look
I deployed to Iraq in 2006 and came home with a wrecked back, a thyroid that never fully came back online, and more concussions than I could count. For fifteen years the standard system managed my symptoms (normal labs, normal follow-ups, normal reassurances) and never ran the panels that would have caught what was actually wrong under the hood.
